c<\/mark>) a  photo of  emergency contact persons<\/mark><\/strong><\/h6>\n\n
d) student’s birth certificate copy<\/mark><\/strong><\/h6>\n\n
e) a copy of parent’s\/guardian’s IC or passport<\/mark><\/strong><\/h6>\n\n
f) child immunization records<\/mark><\/strong><\/h6>\n\n
